To use a compounded medication containing tirzepatide, there are specific criteria that must be met. Your licensed healthcare provider will confirm that the following is true and accurate before prescribing a compounded medication: You must:

1. Be at least 18 years old.
2. Reside in or physically be located in one of the states where services are available.
3. Meet the FDA-approved criteria for these medications, which include having a body mass index (BMI) over 30 for individuals with obesity, and a BMI over 27 with a qualifying health condition such as type 2 diabetes, high blood pressure, or high cholesterol.

4. Provide medical history as requested by your doctor. Certain states/regions require a physical within the last 3 years, including Mississippi, Louisiana, and Puerto Rico.
5. Not have any of the following conditions: Type 1 Diabetes, a history of pancreatitis, severe gastrointestinal (GI) disease, current gallbladder problems (excluding previous gallbladder removal/cholecystectomy), retinal impairment/diabetic retinopathy, bariatric surgery within the past 6 months, multiple endocrine neoplasia syndrome type 2 (MEN-2), or a history of medullary thyroid cancer.
6. Not be pregnant or nursing.
